Embalmers Apprenticeship Agreement (Mentor Agreement) - This form is request initial mentor approval for an apprenticeship application, or to request a mentor change for approval. Apprentices cannot change mentors without approval from the Board. To request a mentor change, please complete this document, pay the applicable $20.00 for each license that you wish to request a mentor change, and submit these to the Board of Registration for review. Initial mentor approval and mentor changes are not guaranteed and are determined by the Board.
